Reminder From the Nurse

1) On or before students’ last day of school, parents/guardians need to stop in to pick up all student medications from the nurse.  Students cannot bring medication home themselves.

2) If your student is entering Pre-K3, Pre-K4, Kindergarten, or 7th Grade next year, per state law they cannot start until we have an updated physical form on file.  If your child falls into one of these categories and has their physical over the summer, please call of email Mrs. Pennington (kpennington@mpspride.org) with the date to leave for the new nurse coming in.  If your student has had a recent physical and you have not submitted the paperwork from the doctor, please do so before school concludes for the year.

3) Also, medication authorization forms need to be renewed at the start of each new school year. It is the parent’s responsibility to bring medication and a completed and signed medication authorization form to the nurse when we return to school in late August. The most recent medication authorization form can be found on the school website under Forms/Docs.
